Please review client forms below. These must be completed before your appointment starts. You may complete them and email prior to your appointment. If you cannot complete them before, please arrive 10 minutes early to your appointment. All form information is confidential.

We ask that you please reschedule or cancel at least 2 days before the beginning of your appointment.

Failure to show up without notice or arrive 10 minutes late will result in you being charged the entire price of your booked service with the card on file. If you exceed the 10-minute grace period, a $50 late charge fee will be applied to your total treatment price. *If you have two or more rescheduled appointments, you will be asked to pay for the service in full before rebooking. *Please note that this will be non-refundable if it is rescheduled. We do not offer refunds or exchanges on products or services, with no exceptions. To uphold the safest environment possible, we request only those receiving services be present in the treatment room.
